At 54 I was diagnosed with COPD stage 2. The Arnhem winters with their damp cold, particulate matter and fluctuating temperatures were destroying my lungs. My pulmonologist seriously advised: "Consider a warmer, drier climate." He gave me brochures about the Costa del Sol, where air quality and constant temperatures are beneficial for lung patients.

I did research and chose Marbella — not the glamour city of the jet set, but the ordinary town behind it. The climate is ideal: average 18 degrees in winter, low humidity, barely any smog thanks to the sea breeze. My lung function has improved by 8% in two years — my Spanish pulmonologist calls it "the climate effect" that he sees in many northern patients.

Healthcare was crucial. Through the convenio especial with the Seguridad Social I pay €60 per month for full public care. My pulmonologist at Hospital Costa del Sol in Marbella specializes in COPD. I have quarterly check-ups, spirometry and a rehabilitation program. My medication — tiotropium and salbutamol — costs €12 per month with the tarjeta sanitaria instead of the €385 health insurance premium plus deductible in the Netherlands.

My house in San Pedro de Alcántara — a quieter neighboring town of Marbella — I rent for €900 per month. Two bedrooms, terrace, ten minutes walk from the beach. The neighborhood is residential, not touristy, with Spanish neighbors and local shops. I shop at Mercadona and the Saturday market. My total monthly expenses are €1,800.

I stopped working early through a WIA disability benefit because of my COPD. The benefit is paid out in Spain and taxed here through the IRPF system. My tax advisor in Marbella calculated that I pay €180 per month less in tax than in the Netherlands. The modelo 720 for foreign assets was important — I had to declare my savings account and investments in the Netherlands.

The best part: I can move again. In Arnhem I lay on the couch for weeks in winter with breathlessness. Here I walk every day, swim three times a week, and do breathing exercises on the beach. My quality of life has improved dramatically. The sunshine, sea air and active outdoor life do more for my lungs than any medication. If your doctor advises a warmer climate: take it seriously. It saved my life.
